Description
Layered radiographic rendering places the greater vestibular (Bartholin) glands in their expected position at the posterolateral margins of the vaginal vestibule, deep to the posterior third of the labia majora and just inferior to the perineal membrane within the superficial perineal pouch. Each gland appears as an ovoid secretory structure whose duct courses anteromedially to open into the vestibule at approximately the 4 and 8 o’clock positions relative to the vaginal orifice, close to the junction with the labia minora. As the sequence progresses, surrounding pelvic landmarks fade in and out to orient the viewer, including the pubic symphysis anteriorly, the ischiopubic rami laterally, and the distal vagina, urethra, and bulbs of the vestibule in the midline. Spatial relationships stay explicit. Clinically, this is the anatomy behind a Bartholin duct cyst and the common progression to a painful abscess when the duct becomes obstructed and infected, often displacing the vestibule and distorting the posterolateral introitus on exam. Animated depth control clarifies why incision and drainage targets the mucosal side of the vestibule and why marsupialization or a Word catheter must follow the ductal trajectory rather than violating the gland, a misstep that increases recurrence and scarring. In patients over 40, the same location matters for identifying a persistent mass that warrants biopsy to exclude Bartholin gland carcinoma, a rare but exam-relevant diagnosis.
