Description
Centered in the anterior superior mediastinum, the thymus appears as paired right and left lobes that meet across the midline behind the manubrium and anterior to the pericardium and great vessels. Across the sequence, a smooth capsular surface resolves into external divisions, with connective tissue septa cleaving the gland into lobules that step inferiorly toward the cardiophrenic angles and, superiorly, toward the lower neck. Subtle asymmetry between the lobes is maintained, with the left lobe typically extending slightly more inferiorly and medially over the pericardial sac. Clinically, this lobular anatomy matters whenever you need to localize thymic tissue in relation to the brachiocephalic veins, aortic arch, and adjacent pleura, because thymectomy for myasthenia gravis or resection of an anterior mediastinal mass depends on staying oriented while dissecting along a thin capsule. Ectopic thymic remnants can track along the cervical descent pathway, and lobulation helps explain why residual tissue may persist in perithymic fat even when the dominant lobes have been removed. Motion clarifies these relationships. A static plate rarely communicates how quickly the thymus narrows, flattens, and changes contour as you move from superior to inferior mediastinum. Use this animation in gross anatomy and immunology teaching to connect thymic microarchitecture to the gross lobular plan, and in thoracic surgery, radiology, or pathology materials that differentiate normal thymus, thymic hyperplasia, thymoma, and other anterior mediastinal differentials on CT or MRI.
