Description
Rotating in three dimensions, the adult male liver is presented with the right lobe occupying the patient’s right upper quadrant and extending medially across the epigastrium toward the smaller left lobe. The falciform ligament courses on the anterior surface, dividing the right and left anatomical lobes as it ascends superiorly toward the diaphragm and continues posteriorly as the coronary and triangular ligaments. As the view progresses, surface landmarks come into relief, including the fissure for the ligamentum teres along the inferior anterior margin and the fissure for the ligamentum venosum on the posterior aspect near the left lobe. Clear boundaries. Attention then shifts inferiorly to the visceral surface where the porta hepatis sits transverse and central, with the expected relationship of portal vein posterior, hepatic artery proper anteromedial, and common hepatic duct anterolateral, while the gallbladder fossa lies to the right of the interlobar fissure and the groove for the inferior vena cava tracks vertically on the posterior right lobe. Segmental geography is implied by these external cues: the caudate lobe (segment I) adjacent to the IVC, the quadrate lobe region between the gallbladder and ligamentum teres, and the broader right and left functional divisions that surgeons follow along Cantlie’s line from the gallbladder bed toward the IVC.
