Description
Centered in the female pelvis, the uterus is shown with its fundus superior, cervix inferior, and the endometrial lining confined to the uterine cavity. Ectopic endometrial-like implants appear on adjacent peritoneal surfaces, classically along the posterior uterus, uterosacral ligaments, and within the rectouterine pouch (pouch of Douglas), with spread toward the ovaries and distal fallopian tubes. As the sequence advances, lesions thicken and darken in keeping with cyclic hemorrhage, and the surrounding serosa becomes hyperemic and irregular. Ovarian involvement progresses into an endometrioma, a cystic space filling with old blood (the typical chocolate cyst appearance). Endometriosis is a leading anatomic correlate of secondary dysmenorrhea, deep dyspareunia, and chronic pelvic pain, and it can distort tubo-ovarian anatomy enough to contribute to subfertility. The animated progression makes the pathology easier to teach: you can track how repeated bleeding drives local inflammation, fibrin deposition, and adhesions that tether the ovary to the pelvic sidewall or posterior uterus, narrowing the cul-de-sac and limiting organ mobility. Pain has a structure. Lesions along the uterosacral ligaments and rectovaginal septum also map to tenderness on bimanual exam and symptoms that flare with menses.
