Description
Beginning at the superior sagittal sinus along the attached margin of the falx cerebri, the sequence tracks venous return posteriorly to the confluence of sinuses and into the paired transverse sinuses coursing laterally within the tentorium cerebelli. Flow continues as each transverse sinus curves inferomedially into the sigmoid sinus, then narrows toward the jugular foramen to form the jugular bulb and internal jugular vein. Deep drainage is integrated as the inferior sagittal sinus joins the great cerebral vein (of Galen) to form the straight sinus, which angles posteriorly to the confluence, while the cavernous sinus connects anteriorly via intercavernous channels and drains posteroinferiorly through the superior and inferior petrosal sinuses. Valves are absent. Directionality matters. Clinical relevance sits in the junctions and bottlenecks: thrombosis in the superior sagittal sinus or transverse sinus can obstruct cortical venous outflow, raising intracranial pressure and producing headache, papilledema, and venous infarction with hemorrhagic conversion.
