Description
Posterior thoracic anatomy is presented in a slightly oblique view, with the trapezius spanning from the external occipital protuberance and nuchal ligament inferiorly to the thoracic spinous processes, then laterally to the acromion and scapular spine. Deep to it, the rhomboid minor and major run from the midline spinous processes to the medial (vertebral) border of the scapula, while the latissimus dorsi sweeps superolaterally from the thoracolumbar fascia toward the humerus, forming the posterior axillary fold. The deltoid caps the lateral shoulder, and the scapular borders sit lateral to the vertebral column over the posterior ribs. Tendinous aponeuroses and fascia read as pale bands against the red muscle bellies. This angle clarifies scapulothoracic mechanics: rhomboids retract and downwardly rotate the scapula, trapezius contributes to upward rotation through its upper and lower fibers, and latissimus dorsi couples trunk movement to humeral extension and adduction. A good teaching view for differentiating sources of interscapular pain, where rhomboid strain and trapezius myofascial trigger points can mimic cervical radiculopathy, and for understanding why long thoracic nerve palsy produces scapular winging (loss of serratus anterior) even when these posterior muscles remain intact. Surface landmarks matter. The medial scapular border and scapular spine guide palpation and safe approaches for posterior shoulder injections and scapular dyskinesis assessment.
