Laterally, the deep cervical fascia is rendered as a continuous investing layer wrapping the neck and sending a distinct fascial process medially to contribute to the carotid sheath. Anterior to this compartment lie the strap muscles and visceral fascia around the larynx and trachea, while posteriorly the prevertebral fascia invests the cervical vertebrae and deep neck muscles. Superiorly the fascia blends toward the skull base and mandibular region, and inferiorly it descends toward the thoracic inlet where fascial planes become clinically contiguous with the superior mediastinum. Understanding the process of the deep cervical fascia around the carotid sheath matters because it frames how infection, hematoma, and air track through the neck. Deep neck space infections can spread along these fascial boundaries from odontogenic or tonsillar sources into the parapharyngeal space, and in severe cases descend toward the mediastinum.
