Description
Arising from the proper hepatic artery within the hepatoduodenal ligament, the left hepatic artery courses superiorly and to the patient’s left before entering the porta hepatis, where it distributes segmental branches through the left lobe. A semi-transparent hepatic parenchyma allows the intrahepatic arterial tree to be followed alongside the portal vein and its left portal branch, with the artery remaining anterior to the portal venous channels in the typical portal triad arrangement. More proximal caliber change from artery to smaller arterioles is implied as vessels taper toward the peripheral segments. Depth cues and shading clarify the left lobe’s relationship to the midline and the fissure region where hilar structures converge. Left hepatic arterial anatomy matters because it is one of the common sites of clinically relevant variation, and small differences in origin or course can change the risk profile during cholecystectomy, lymphadenectomy along the common hepatic artery, or hilar dissection. Surgeons planning left hepatectomy or living-donor transplantation need a clear mental model of where arterial inflow enters the left hemiliver relative to the portal vein, since arterial injury or thrombosis can produce biliary ischemia even when portal inflow appears intact.
