Description
From a posterior perspective, the fibular (lateral) collateral ligament (LCL) is seen as a distinct cord running from the lateral femoral epicondyle inferiorly to the fibular head, lying superficial to the posterolateral joint capsule rather than blending with it. Laterally, the fibular head sits distal and slightly posterior to the lateral tibial plateau, providing the LCL’s firm distal attachment while the biceps femoris tendon approaches the same region from a more proximal and posterior direction. Deeper in the intercondylar region, the posterior cruciate ligament courses from the posterior tibia to the medial femoral condyle, with the anterior cruciate ligament crossing anterior to it; both sit medial to the LCL and are framed by the posterior margins of the femoral condyles. Clear separation. That posterior-lateral relationship matters when you are teaching or planning around posterolateral corner injury patterns, where varus stress and external rotation can disrupt the LCL and adjacent structures while leaving the cruciate ligaments intact, or vice versa in multiligament trauma. Clinically, the LCL’s extracapsular course helps explain why isolated LCL sprains may present with localized fibular head tenderness and varus laxity at 30 degrees of knee flexion, yet lack the effusion you often see with intracapsular cruciate tears. This is the view that makes that distinction easy to explain.
