Arising from the pulmonary trunk, the left pulmonary artery courses laterally and slightly posteriorly to the left of the ascending aorta before passing superior to the left main bronchus and dividing into lobar and segmental branches within the hilum. From a posterior perspective, the vessel arborization is seen descending into the upper and lower lobes, tracking alongside the bronchial tree as it narrows into smaller pulmonary arteries and arterioles toward the peripheral lung. Posterior segmental territories are emphasized, where arterial branches follow segmental bronchi into the dorsal parenchyma. Orientation is clear. Clinical relevance sits at the level of the segmental branches. Emboli that lodge at a segmental pulmonary artery can produce wedge-shaped, pleural-based pulmonary infarction, and a posterior view helps correlate the distribution of perfusion defects on CT pulmonary angiography or nuclear medicine V/Q studies with the bronchopulmonary segments involved.
