Deltopectoral Groove

Description

Running inferior to the clavicle, the deltopectoral groove forms a clear sulcus between the anterior border of the deltoid laterally and the superior fibers of pectoralis major medially. At its superior apex near the infraclavicular fossa, the interval blends with the deltopectoral (clavipectoral) triangle, where the clavipectoral fascia spans deep to the groove toward the coracoid process. Deep to this superficial landmark, the cephalic vein typically ascends within the groove before piercing the clavipectoral fascia to join the axillary vein, with the deltoid branch of the thoracoacromial artery and regional lymphatics in the same corridor. Clean surface anatomy. For procedural anatomy, the deltopectoral groove matters because it is a reliable cutaneous guide to the cephalic vein and the deltopectoral approach, used for shoulder arthroplasty, proximal humerus fracture fixation, and open reduction around the bicipital groove without splitting the deltoid. Venous access via cephalic vein cutdown for pacemaker or defibrillator lead placement commonly exploits this interval, and variant cephalic anatomy or a prominent deltopectoral fat pad can affect exposure and cannulation. Knowledge of the clavipectoral fascia planes also helps explain how superficial infection or hematoma can track from the chest wall toward the axilla.
