Running transversely across the palmar aspect of the metacarpophalangeal region, the distal commissural ligament (deep transverse metacarpal ligament) forms a stout white band linking the volar plates at the level of the metacarpal heads, spanning from the ulnar side of the index ray toward the radial side of the little finger. Just superficial to it lie the palmar fascia and subcutaneous tissue, while the flexor tendons in their synovial sheaths course distally toward the digits, flanked by the lumbricals and the digital neurovascular bundles along the radial and ulnar borders. Deep to the ligament, the interossei occupy the intermetacarpal spaces and approach the proximal phalanges, with the metacarpal heads positioned dorsally relative to the volar plates and transverse band. Palmar exposure of this structure matters when you need to explain why the finger webs maintain their spacing and why the flexor tendons remain centrally aligned at the metacarpal level during grasp. It is a key restraint against excessive splaying of the digits, and it becomes a practical landmark during volar approaches to the metacarpophalangeal joints, where dissection must respect the adjacent digital arteries and nerves and the A1 pulley region to avoid postoperative stiffness or sensory disturbance.
