Description
Posterior uterine anatomy is rendered with emphasis on the facies intestinalis, where the peritoneal serosa reflects over the convex fundus and upper corpus. Superolaterally, the uterine tubes emerge at the cornua and project laterally, establishing the junctional landmarks where the fundus transitions into the tubal angles. The posterior surface sweeps inferiorly toward the isthmus and cervix, a relationship that frames the rectouterine pouch (pouch of Douglas) between the uterus and rectum. Orientation is clear. This posterior perspective matters when teaching how the uterus relates to bowel and peritoneal spaces, because the facies intestinalis is the surface most often involved in adhesions that tether the uterus to the rectosigmoid after pelvic inflammatory disease, endometriosis, or prior surgery. It also supports operative planning: posterior cul-de-sac obliteration and uterosacral ligament tension are assessed from behind during laparoscopy and influence approaches to hysterectomy, uterine suspension, and deep infiltrating endometriosis excision.
