Description
Superiorly in the right hemithorax, the apex of the right lung rises above the level of the first rib and is capped by cervical pleura. The apical (S1) bronchopulmonary segment occupies the most superomedial portion of the right upper lobe, lying superior to the posterior segment (S2) and anterior segment (S3). Segmental boundaries are defined by intersegmental pulmonary veins, while the segmental bronchus (B1) and its accompanying branch of the right pulmonary artery course centrally within the segment. Clear orientation at the apex matters. Apical segment anatomy is a frequent point of confusion because the apex is also a surface landmark, and it sits adjacent to structures that drive real clinical decisions, including the subclavian vessels and lower trunk of the brachial plexus at the thoracic inlet.
