Description
Inferior orientation of the heart frames the left ventricle (ventriculus sinister) as the dominant chamber, with its thick myocardium forming the diaphragmatic surface. The inferior wall lies posterior to the sternocostal surface and rests adjacent to the central tendon of the diaphragm, while the interventricular septum sits medial to the left ventricular free wall. Along the basal margin, the left ventricular chamber approaches the atrioventricular junction where the mitral annulus would be located, and the apex projects inferolaterally in anatomical position. Clinically, this inferior positional arrangement matters because it maps directly onto the so called inferior (diaphragmatic) myocardial infarction territory, most often related to right coronary artery supply in a right dominant circulation, with important contributions from the posterior descending artery and, variably, the left circumflex. Inferior wall ischemia can involve the posteromedial papillary muscle, predisposing to acute mitral regurgitation, and it often coincides with conduction system effects through proximity to the AV node region. A hard wall, but a vulnerable neighborhood. Use this asset for gross anatomy and cardiothoracic modules when teaching chamber orientation, surface anatomy, and the relationship between ventricular myocardium and the diaphragm, or for cardiology content explaining inferior STEMI patterns on ECG (II, III, aVF) and correlating them with coronary dominance. It also suits surgical education discussing exposure of the diaphragmatic surface during cardiac positioning and posterior pericardiotomy approaches.
