Arising from the conchal cartilage, the antihelix forms a prominent Y-shaped ridge on the lateral surface of the male auricle, positioned posterior and medial to the helix and separated from it by the scaphoid fossa. Superiorly, the antihelix divides into the superior and inferior crura that bracket the triangular fossa, while inferiorly it courses toward the antitragus and the incisura intertragica near the external acoustic meatus. The concha lies anteromedial to this relief, funneling toward the cartilaginous portion of the external auditory canal. Lateral appreciation of the antihelix matters because many external ear interventions depend on recognizing these cartilaginous landmarks by surface contour, not by dissection. Otoplasty for prominent ear commonly reshapes or recreates the antihelical fold with Mustarde sutures, and the surgeon’s target is the relationship between the antihelix, scapha, and helix rim.
