Description
Posteriorly oriented anatomy of the right intermediate bronchus is presented between the takeoff of the right upper lobar bronchus superiorly and the origin of the right middle and right lower lobar bronchi inferiorly. From this rear perspective, the bronchus intermedius runs inferiorly and slightly laterally from the right main bronchus toward the hilum, with its lumen narrowing as it approaches the lobar bifurcations. Segmental bronchi to the lower lobe arise distally, while the superior lobar bronchus sits more superior and anterior relative to the intermediate trunk. Short and deceptively simple. Bronchoscopy and endobronchial procedures depend on this geometry because the bronchus intermedius functions as the common conduit to the middle and lower lobes, and subtle posterior angulation can make the distal carina easy to miss when advancing a scope. Right-sided aspiration events often track into the bronchus intermedius and then into the lower lobe bronchi, and obstructing lesions here can produce combined right middle and right lower lobe atelectasis with a single point of narrowing. This view also supports teaching the relationship of lobar and segmental branching patterns that underlie the nomenclature of bronchial segments and surgical segmentectomy planning.
