Description
Seen from the side, the maternal torso is rendered semi-transparent so the gravid uterus fills the abdomen from the pelvic inlet to the level of the costal margin, its fundus displacing the intestines superiorly and posteriorly. Within the amniotic cavity the term fetus lies in cephalic presentation, the vertex engaged in the maternal pelvis with the spine curved anteriorly, hips and knees flexed and the arms drawn toward the chest. The umbilical cord loops between the fetal abdomen and the uterine wall, while the placenta is implanted along the uterine surface. Adjacent maternal structures remain visible through the overlay, including the urinary bladder compressed anteroinferiorly, the rectum and sigmoid colon posteriorly, the bony pelvis and the enlarged breast. At 38 weeks the pregnancy is at early term, and this lateral projection shows the anatomical relationships that determine obstetric management: fundal height, the degree of engagement of the presenting part, and the crowding of the bladder and rectum that explains urinary frequency, constipation and pelvic pressure late in gestation. The upward displacement of the diaphragm and abdominal viscera underlies maternal dyspnoea and reflux, while the longitudinal lie with vertex presentation is the configuration assessed by Leopold manoeuvres and by ultrasound before labour. Comparison with earlier stages in the series makes the progressive change in uterine volume and fetal posture explicit. The image suits antenatal education leaflets, midwifery and obstetric nursing curricula, and textbook figures on fetal lie, presentation and engagement. It also serves clinicians counselling patients on late-pregnancy symptoms, childbirth preparation classes, and publishers or app developers assembling week-by-week gestational timelines. Anatomical accuracy verified by SciePro's Medical Advisory Board.
