Oriented in a right lateral profile, the right lung is rendered with the medial segment of the middle lobe (segmentum mediale, S5) color-coded against the remaining upper, middle, and lower lobes. The highlighted segment occupies the anteromedial portion of the lobus medius, lying inferior to the horizontal fissure and anterior to the oblique fissure, with its contour tapering toward the anterior border and the region of the cardiac impression. Superiorly it relates to the anterior segment of the upper lobe, while posterior and inferior neighbors transition toward basal segments of the lower lobe along the oblique fissure. Fissural anatomy frames the segment. Segmental localization of S5 matters because the right middle lobe is a frequent site of recurrent atelectasis and post-obstructive change, often discussed under right middle lobe syndrome when impaired drainage or external compression involves the bronchus.
