Description
Obliquely from the lateral neck, the anterior cervical region is organized around the muscular triangle (trigonum musculare, omotracheal triangle), bounded anteriorly by the median line of the neck, posterolaterally by the anterior border of the sternocleidomastoid muscle, and inferiorly by the superior belly of the omohyoid as it courses anteromedially. Within this space, the infrahyoid strap muscles, sternohyoid and sternothyroid, lie superficial and medial, while the thyrohyoid sits superiorly approaching the hyoid bone. Deeper, the laryngeal framework and upper trachea occupy the medial compartment, with the thyroid gland expected anterolateral to the trachea and inferior to the thyroid cartilage. A side appraisal of this triangle matters because it clarifies what is and is not accessible through an anterior neck approach without entering the carotid triangle. Surgeons and trainees use the omohyoid and sternocleidomastoid as surface-to-deep guides when planning a tracheostomy or thyroidectomy exposure, where the midline raphe between strap muscles is opened to reach the tracheal rings while avoiding the carotid sheath laterally and the anterior jugular veins in the superficial fascia. Small errors here become bleeding and airway problems.
