Description
Arising from the common tendinous ring at the orbital apex, the superior rectus muscle courses anteriorly along the superior surface of the globe, lying deep to the levator palpebrae superioris and superior to the optic nerve. Its tendon fans out to insert on the sclera just posterior to the corneoscleral junction (limbus), placing the insertion anterior to the equator of the eyeball and medial to the superotemporal quadrant. A cross-section through the orbit would also place the muscle immediately superior to the superior oblique tendon as that tendon turns through the trochlea anteromedially. Clean, teachable geometry. Clinically, the superior rectus matters because its action profile shifts with globe position: it elevates most efficiently in abduction, while adduction recruits more intorsion and adduction from the rectus pulley system. That distinction helps localize restrictive disease such as thyroid eye disease, where fibrotic enlargement can limit elevation and mimic or mask neurogenic patterns. The close relationship to the superior ophthalmic vein and orbital roof also makes it a frequent landmark during superior transconjunctival and upper eyelid approaches, and its anterior scleral insertion is relevant when planning strabismus recession or resection.
