Description
Running from the anterior superior iliac spine to the pubic tubercle, the inguinal ligament forms a thickened inferior border of the aponeurosis of the external oblique muscle and spans the anteroinferior margin of the male pelvis. Medial fibers reflect posteriorly as the lacunar ligament to the pecten pubis, while deeper reinforcement from the transversalis fascia and iliopubic tract lies posterior and superior to the ligament, shaping the floor of the inguinal canal. Superior to it, the internal oblique and transversus abdominis contribute arching fibers that define the canal’s roof, and inferior to it the femoral vessels pass beneath the ligament toward the femoral triangle. Bony landmarks of the ilium, pubis, and ischium anchor the relationships. A key landmark. Oblique visualization of the inguinal ligament matters because it clarifies the handoff between abdominal wall and thigh, the same anatomic junction that drives both hernia formation and operative repair. Indirect inguinal hernias track lateral to the inferior epigastric vessels through the deep inguinal ring, while direct hernias push through Hesselbach’s triangle just medial to those vessels, both conditions relying on the integrity of the posterior wall (transversalis fascia, conjoint tendon) relative to the ligament’s inferior margin. Surgeons also use the pubic tubercle, pectineal (Cooper’s) ligament, and the lacunar ligament as tactile guides during open and laparoscopic mesh fixation, where a few millimeters can separate safe placement from injury to the femoral vein or an aberrant obturator artery. Educators can drop this artwork into gross anatomy labs when teaching the anterior abdominal wall, or into surgical clerkship materials on inguinal and femoral hernia anatomy, repair planes, and surface landmarking in the male groin.
