Description

Arising from the superior border of the scapula, the inferior belly of the omohyoid runs anterosuperomedially across the lower lateral neck, then continues as an intermediate tendon tethered to the deep cervical fascia before the superior belly ascends to the inferior border of the hyoid bone. Relative to the sternocleidomastoid, the omohyoid courses deep in its lower segment and becomes more superficial as it approaches the hyoid, forming part of the anterior boundary of the posterior triangle. The superior belly lies lateral to the sternohyoid and inferior to the submandibular region. Two bellies, one tendon. That fascial linkage is the teaching point. When the omohyoid contracts it tensions the cervical fascia and can narrow the internal jugular vein, a detail that matters during central venous catheterization and neck dissection when venous distension or unexpected tethering complicates exposure. Surgeons also use the omohyoid as a practical landmark when mapping the levels of cervical lymph nodes, since it crosses the carotid sheath region and helps orient the superior and inferior extents of selective neck dissection fields. Use this illustration in gross anatomy and head and neck modules to clarify strap muscle layering, triangle boundaries, and the relationship of the omohyoid to the hyoid and scapular attachments in a male specimen.
