Description
Arising from the costocervical trunk, the deep cervical artery ascends in the posterior neck deep to the semispinalis capitis and splenius muscles, tracking superiorly along the laminae and spinous processes of the lower cervical vertebrae. From this posterior perspective, its course sits lateral to the midline over the vertebral column, with accompanying deep cervical veins returning alongside it. Posterior rami of the cervical spinal nerves and the occipital nerve field are shown in close proximity, crossing the paraspinal compartment as they emerge from the intervertebral foramina. Bony landmarks anchor the scene, including the occipital bone, cervical vertebrae, and the medial border of the scapula. For posterior cervical approaches, this artery is the one you want to remember. Dissection in the suboccipital and deep paraspinal planes can encounter the deep cervical artery as it supplies the semispinalis cervicis and capitis, and an unrecognized branch can bleed briskly in the narrow operative corridor. Its anastomoses with the occipital artery and ascending cervical artery also explain why posterior neck vascularity can remain robust even when one feeder is compromised, a point that matters when planning flaps or interpreting postoperative hematoma patterns.
