Description
Running along the anterior forearm, the flexor carpi radialis muscle belly arises from the medial epicondyle of the humerus as part of the common flexor origin, then tapers distally into a long, palpable tendon that sits lateral to the palmaris longus and medial to the radial artery near the wrist. Proximally, the brachialis occupies the anterior aspect of the distal humerus and inserts on the ulnar tuberosity, forming the muscular floor of the cubital fossa deep to the superficial flexor mass. Radius and ulna flank the elbow joint, while the carpal bones and metacarpals align distally beneath the flexor tendons at the volar wrist. Clear bony landmarks anchor the relationships. Clinically, the anterior course of the flexor carpi radialis tendon matters because it runs in its own fibro-osseous tunnel at the wrist and inserts at the bases of the second and third metacarpals, so pain and crepitus along this cord often point to flexor carpi radialis tendinopathy rather than carpal tunnel syndrome. Surgeons also use the FCR tendon as a reliable landmark for the volar (Henry) approach to the distal radius and for exposure during scaphoid or radiocarpal procedures; staying ulnar to the tendon helps avoid the radial artery, which lies immediately lateral. Common confusion arises at the wrist crease. This view clears it up.
