Description
Seen from an inferior (perineal) perspective, the male pelvic floor is framed laterally by the bony pelvis and the obturator internus lining the pelvic wall on each side. Medial to obturator internus, the levator ani is represented as a broad muscular sling, with the puborectalis forming the most medial U shaped loop that passes posterior to the anorectal junction and blends with its contralateral partner to create a muscular collar. Anteriorly, the urogenital hiatus accommodates the urethra, while the anal canal lies posteriorly, cradled by the puborectalis at the level where the anorectal angle is maintained. Puborectalis anatomy matters because continence is not just a sphincter story, it is geometry. By pulling the anorectal junction anteriorly toward the pubic symphysis, puborectalis sharpens the anorectal angle, and relaxation during defecation permits straightening, a relationship that underpins both digital rectal exam teaching and anorectal physiology. Disruption of levator ani, whether from iatrogenic injury during abdominoperineal or low anterior resection, pudendal neuropathy, or chronic straining with levator ani syndrome, can reduce this sling effect and contribute to fecal incontinence, obstructed defecation, and pelvic floor descent.
