Description
Anterior exposure of the male knee joint without the patella (capless genu) brings the distal femur and proximal tibia into full view, with the medial and lateral femoral condyles positioned superior to the tibial plateau. Between the condyles, the intercondylar notch opens centrally, while the smooth hyaline articular cartilage caps the femoral condyles and the corresponding tibial articular surfaces inferiorly. Medially, the larger medial condyle projects slightly farther distally than the lateral condyle, and the intercondylar eminence of the tibia rises between the tibial condyles where the cruciate ligaments would attach. No patellofemoral articulation is present in the field because the patella and trochlear engagement are intentionally removed. Patellar removal clarifies femorotibial congruence and the geometry of the intercondylar region, a common blind spot when the extensor mechanism dominates the anterior view. Surgical relevance is immediate: arthrotomy, total knee arthroplasty exposure, and notchplasty planning depend on understanding how the intercondylar notch relates to the femoral condyles and tibial spines, and how much cartilage remains at the load-bearing zones.
