Curving across the deep plantar aspect of the male forefoot, the deep plantar arch is formed primarily by the lateral plantar artery joining the deep plantar branch of the dorsalis pedis, then sweeping medially beneath the bases of the metatarsals. Plantar metatarsal arteries arise from this arcade and course distally toward the metatarsophalangeal joints, where they contribute to digital perfusion through plantar digital branches. Surrounding osseous landmarks include the tarsals (talus and calcaneus) proximally and the metatarsals and phalanges distally, with articular cartilage surfaces indicated at the interphalangeal and metatarsophalangeal articulations. Deep location matters here. The arch lies plantar to the interossei and close to the plantar metatarsal spaces, so it is exposed during plantar or intermetatarsal approaches used for drainage of deep forefoot infection, excision of Morton neuroma, or management of metatarsal fractures and Lisfranc-region instability. Vascular compromise at this level has direct clinical consequences in diabetes and peripheral arterial disease, where reduced inflow from the dorsalis pedis or posterior tibial system can translate into poor digital wound healing and plantar ulcer risk, and where palpation fails, angiographic planning often hinges on understanding the dorsalis pedis to deep plantar connection.
