Description
Posterior shoulder girdle musculature of a male child is presented with layered myology of the upper back, scapula, and proximal arm. Superficially, the trapezius spans from the nuchal region and thoracic midline to the spine of the scapula and lateral clavicle, while the deltoid caps the glenohumeral joint laterally. Deep to this plane, the rhomboid major and minor run inferolaterally from the spinous processes to the medial border of the scapula, the infraspinatus occupies the infraspinous fossa posterior to the scapular spine, and teres major courses from the inferior angle toward the medial lip of the intertubercular sulcus, blending with latissimus dorsi inferiorly. Fiber direction is clear. That posterior-lateral scapular region is where teaching and clinical anatomy intersect, because scapular dyskinesis often reflects imbalance between the trapezius and rhomboids and altered rotator cuff mechanics at the glenohumeral joint. In pediatric and adolescent patients, traction or direct blows can produce scapular winging patterns that differ by lesion level: long thoracic nerve involvement affects serratus anterior (not seen here), while spinal accessory nerve palsy flattens the upper trapezius contour and shifts the scapula laterally and inferiorly. The relationship of infraspinatus and teres minor to the posterior humeral head also grounds discussions of posterior shoulder dislocation and rotator cuff strain from overhead sports, even before adult muscle bulk develops.
