Description

Posterior musculature of an adult female is rendered with emphasis on the semitendinosus within the medial hamstring compartment. Arising from the ischial tuberosity, the semitendinosus belly lies medial to the long head of biceps femoris and superficial to semimembranosus, then narrows distally into a long, cord-like tendon that passes posterior to the medial femoral condyle toward the proximal medial tibia. Adjacent landmarks include the gluteus maximus superiorly, the popliteal fossa centrally, and the medial knee where the semitendinosus tendon approaches the tendons of gracilis and sartorius (pes anserinus). Clear depiction of the semitendinosus tendon matters because it is a frequent autograft source for anterior cruciate ligament reconstruction, harvested through a small anteromedial tibial incision where the pes anserinus tendons can be misidentified. Small mistakes here are common. This posterior view also supports teaching of proximal hamstring pathology, where tendon origin at the ischial tuberosity is implicated in chronic tendinopathy and acute avulsions, and helps orient the clinician to the relationship of the hamstring mass to the popliteal fossa during physical examination and surgical positioning. Use this illustration in gross anatomy and kinesiology teaching to compare the medial and lateral hamstrings, trace fiber direction, and correlate knee flexion and hip extension with tendon trajectory. It also fits well in orthopedic and sports medicine materials describing hamstring strain patterns, graft selection for ACL surgery, and the anatomy of the pes anserinus and medial knee pain syndromes.
