Description
Posteriorly, the semi-transparent liver is opened to the venous tree within its parenchyma, with intrahepatic venules converging into the major hepatic veins as they course superiorly and medially toward the caval confluence. The right hepatic vein typically drains the right lobe and approaches the inferior vena cava on the posteromedial surface, while the middle and left hepatic veins run in the planes of the main portal fissure and left intersegmental fissure before joining near the diaphragm. Blue venous trunks cluster along the deep groove for the inferior vena cava, immediately posterior to the bare area and superior to the caudate lobe. Clear landmarks. This posterior angle matters because it mirrors the surgical and radiologic logic of hepatic outflow, where segmental drainage patterns dictate safe transection planes in hepatectomy and living-donor graft procurement. Hepatic venous outflow obstruction (Budd-Chiari syndrome) localizes to these large hepatic veins or their junction with the inferior vena cava, and the distribution of congestion is best understood when you can track where the small venules coalesce into named trunks. Interventional access also follows this anatomy, since transjugular hepatic venography and TIPS target hepatic veins near their caval entry.
