The midsagittal cut reveals endometriotic implants on multiple pelvic surfaces, demonstrating how the disease spreads beyond the uterus to involve the rectouterine pouch, bladder peritoneum, and bowel wall. Deep infiltrating endometriosis (DIE) is the most severe form of the disease, penetrating more than 5 millimeters below the peritoneal surface. Lesions on the bladder cause urinary symptoms, while rectal and sigmoid implants produce bowel symptoms that overlap with other GI conditions.
