A 3D model of the male small intestine showing a W-pouch configuration. Four limbs of terminal ileum are arranged in a W-shape and anastomosed to create the largest reservoir volume of the standard pouch designs. The pouch connects to the anal canal for fecal continence. The W-pouch maximizes reservoir capacity, which can reduce stool frequency compared to smaller pouch designs. Its construction is technically demanding and uses more ileum, but it may benefit patients who need maximum reservoir volume.
