Description
A 3D cross-section model of a blackhead (open comedone) showing the dilated follicular infundibulum filled with compacted keratin, sebum, and bacteria. The wide opening at the skin surface allows oxidation of the surface material, producing the dark color. Open comedones form when the sebaceous follicle becomes plugged with desquamated keratinocytes and sebum, but the follicular ostium remains patent. The dark appearance is caused by melanin oxidation and lipid oxidation at the exposed surface, not by dirt. The dilated pore may reach several millimeters. Open comedones are less inflammatory than their closed counterparts because the open channel allows some drainage. Comedone pathogenesis is the starting point for acne education in dermatology.
