Description
Coiled within the female abdominal cavity, the small intestine is traced from the duodenum in the upper abdomen, where it forms its C-shaped curve around the head of the pancreas, into the jejunum and ileum suspended by the mesentery. As the sequence progresses, jejunal loops tend to occupy the left upper and central abdomen while ileal loops drift more inferiorly and to the right, approaching the ileocecal junction in the right lower quadrant. The root of the mesentery runs obliquely from the duodenojejunal flexure near the ligament of Treitz down toward the cecum, and the moving loops repeatedly cross this line as they settle against the anterior abdominal wall, pelvic brim, and posterior parietal peritoneum. Motion clarifies the three-dimensional crowding. Loop orientation is not just topography, it frames how clinicians think about obstruction and inflammatory disease. A closed-loop small-bowel obstruction from an adhesion often produces a clustered, radially oriented group of dilated loops converging toward a fixed point in the mesentery, a pattern you can understand faster when the bowel is seen shifting in relation to the mesenteric attachment. Crohn disease frequently targets the terminal ileum, so the animated continuity from jejunum to ileum helps learners connect right lower quadrant pain, creeping fat, and fistulizing tracks to the distal small bowel and its proximity to the cecum and pelvis, where endometriosis and postsurgical adhesions also complicate presentations.
