Description
Uterus and adnexa are presented with both ovaries positioned in close apposition in the posterior cul-de-sac, the classic “kissing ovaries” configuration. The ovaries lie posterior and slightly inferior to the uterine corpus, approximated across the midline behind the cervix and uterosacral ligament region, where adhesions often tether the ovarian surfaces and restrict normal lateral separation. Fallopian tubes are expected to be draped superiorly from the uterine cornua toward the ovarian poles, while the surrounding pelvic cavity emphasizes how little space exists between posterior uterine serosa and the rectouterine pouch (pouch of Douglas). Adhesions dominate the spatial story. Kissing ovaries most often point to deep infiltrating endometriosis, prior pelvic inflammatory disease, or postoperative scarring, and the configuration is closely associated with obliteration of the cul-de-sac and concomitant rectovaginal or uterosacral involvement. That matters when you plan laparoscopy: adherent ovaries fixed posterior to the uterus raise the risk of difficult ovarian mobilization, bleeding from the utero-ovarian ligament and infundibulopelvic region, and occult bowel adherence in the rectouterine space. The fixed, posterior relationship shown here supports teaching of why transvaginal ultrasound may report “ovaries stuck together” and why MRI often documents posterior compartment disease when the ovaries are medialized behind the uterus. Hard to ignore. Use this illustration in gynecologic anatomy modules on pelvic peritoneal recesses, in reproductive medicine discussions of endometriosis-associated infertility, and in surgical consent or patient education materials explaining adhesiolysis and posterior compartment dissection. It also suits radiology-pathology correlation in endometriosis conferences, where the midline approximation of the ovaries is a recognizable sign.
