Description
Running superiorly within the carotid sheath, the common carotid artery ascends in the anterior neck and divides near the upper border of the thyroid cartilage into the internal carotid artery (posterolateral) and external carotid artery (anteromedial). The internal jugular vein lies lateral to the artery, while the vagus nerve tracks posteriorly between them. Superficial to the sheath, the sternocleidomastoid muscle crosses obliquely, and the thyroid gland sits anterior and inferior to the bifurcation with the trachea in the midline and the esophagus slightly posterior. Anterior orientation makes the carotid bifurcation easy to localize relative to palpable landmarks, a detail that matters in carotid endarterectomy planning and in duplex ultrasound correlation of atherosclerotic plaque at the carotid bulb. The external carotid’s early branches, often represented in this kind of view, help explain why bleeding patterns differ between facial and intracranial territories, while nearby cervical sympathetic fibers and the hypoglossal nerve (CN XII) frame the risk profile of high carotid exposure. A critical landmark.
