Description
Captured in a medial profile of the right ankle and hindfoot, the talocrural (ankle) region is centered on the distal tibia and its medial malleolus as they articulate with the trochlea of the talus. Inferior and slightly posterior to the talus, the calcaneus outlines the posterior heel, while the medial longitudinal arch continues anteriorly toward the navicular and medial cuneiform. A blue elastic support encircles the ankle just proximal to the malleolar level, crossing the soft-tissue envelope that overlies the deltoid ligament and the tarsal tunnel corridor. Medial ankle anatomy matters because pain and instability here are rarely simple. The deltoid ligament complex resists valgus tilt and external rotation of the talus, and injury can accompany syndesmotic sprains or occur with ankle fractures where medial tenderness changes management. Just posterior to the medial malleolus run the tibialis posterior, flexor digitorum longus, and flexor hallucis longus tendons with the posterior tibial neurovascular bundle, a tight space where posterior tibial tendon dysfunction or tarsal tunnel syndrome can present, and where compression from bracing or swelling becomes clinically relevant.
