Centered on the posterior midline, the intergluteal cleft (natal cleft, rima ani) separates the right and left gluteal regions as it descends from the sacral area toward the perineum. Superiorly, the groove aligns with the median sacral crest and overlies the lower sacrum, continuing inferiorly toward the coccyx and anal region. The gluteal contours sit lateral to the cleft, with the skin and subcutaneous tissue forming the visible depression between the gluteus maximus masses. A narrow highlighted line tracks this midline course. An accurate posterior view of the intergluteal cleft matters because many midline pathologies declare themselves here, and small deviations in location change your differential.
