Posterior abdominal and pelvic viscera are arranged around the vertebral column, with the paired kidneys lying retroperitoneally on either side of the lumbar spine and capped superiorly by the suprarenal (adrenal) glands. Inferior poles of the kidneys sit superior to the iliac crests, while the descending colon courses along the left posterior abdominal wall lateral to the left kidney before curving into the sigmoid colon and continuing to the rectum in the midline pelvis. Superiorly, the upper abdominal organs are partially included, with the stomach to the left and the liver to the right, both positioned anterior to the retroperitoneal structures and separated from the posterior body wall by the peritoneal cavity. A posterior perspective clarifies what is retroperitoneal versus intraperitoneal, a distinction that guides both diagnosis and operative planning. Kidney and adrenal pathology, including perinephric abscess, retroperitoneal hemorrhage, or adrenal mass effect, spreads along fascial planes behind the parietal peritoneum, while colonic disease such as diverticulitis of the sigmoid colon threatens the pelvis and can tether or inflame adjacent retroperitoneal tissues. It also reflects the surgical logic of mobilizing the descending colon along the white line of Toldt to access the left kidney or ureter, and the way posterior abdominal pain can localize despite deep visceral origin. Orientation matters. Use this artwork for teaching posterior abdominal wall relationships in gross anatomy, abdominal imaging correlation (CT and MRI often require mental reconstruction of these compartments), and for surgical or urologic publications discussing retroperitoneal approaches, colectomy mobilization, or pelvic colorectal disease. It also reads well in patient education materials when explaining why kidney symptoms and bowel symptoms can overlap in location but not in compartment.
