Description
Seen in lateral profile on a standing adult female, the abdominal wall is opened conceptually to emphasize the internal oblique muscle and its characteristic superomedial fiber direction, lying deep to the external oblique and superficial to the transversus abdominis. Superiorly, its fibers arise from the thoracolumbar fascia and lower ribs, while inferiorly they sweep from the iliac crest and inguinal ligament toward the linea alba and pubic crest. Posteriorly the muscle thickens toward the flank, and anteriorly it blends into the rectus sheath where it contributes to the anterior and posterior laminae above the arcuate line. Other major superficial muscle groups remain visible as context, with ribs, vertebral column, and pelvis faintly indicated beneath the musculature. A clean teaching view. Understanding the internal oblique in relation to the external oblique and transversus abdominis matters when you are teaching layered abdominal wall anatomy and when you are planning access through the lateral abdomen. The internal oblique and transversus form the muscular roof of the inguinal canal and give rise to the cremaster muscle in females as a small remnant, so weakness or denervation can influence the mechanics of indirect inguinal hernia and postoperative bulge after flank incisions. It is also the key muscular plane for a transversus abdominis plane (TAP) block, where local anesthetic is deposited between internal oblique and transversus to target the anterior rami of T7 to L1.
