Description

Posterior to the knee joint, the female popliteal fossa is framed superolaterally by the biceps femoris tendon and superomedially by the semimembranosus with the semitendinosus lying more superficial and medial. Inferolaterally and inferomedially, the lateral and medial heads of gastrocnemius converge toward the calcaneal tendon, with plantaris often crossing obliquely in the superficial layer. Deep to these margins, the floor is formed by the popliteal surface of the femur superiorly, the posterior capsule and oblique popliteal ligament centrally, and the popliteus muscle over the proximal tibia inferiorly. Key neurovascular structures occupy the midline corridor. Centrally, the popliteal artery runs deepest against the capsule, the popliteal vein lies superficial to it, and the tibial nerve is most superficial, a relationship that matters when you are teaching safe dissection planes or explaining why vascular injury can occur during posterior capsulotomy. The common fibular (peroneal) nerve tracks laterally along the biceps femoris tendon toward the fibular head, a frequent site of neuropraxia with lateral knee trauma and an anatomic reason foot drop may follow fibular neck fractures. Baker (popliteal) cysts distend from the gastrocnemius semimembranosus bursa and present as a posteromedial mass that can mimic deep vein thrombosis, while true popliteal artery aneurysms or entrapment syndromes produce exertional calf pain and distal ischemic signs.
