Description
Viewed laterally, the T12 vertebra sits at the thoracolumbar junction, with its vertebral body positioned anterior to the vertebral arch and its spinous process projecting posteriorly into the thoracic kyphosis. Superior and inferior endplates bound the body, separated from adjacent vertebrae by the intervertebral discs, while the pedicles and laminae frame the vertebral foramen posterior to the body. Facet (zygapophyseal) joints are oriented in transition between thoracic and lumbar patterns, and the costal facet anatomy is reduced compared with typical mid thoracic levels. A boundary level. Clinically, this segment matters because T12 is a common site of junctional injury and degenerative change where the relatively rigid rib-bearing thoracic spine meets the more mobile lumbar spine. Burst and wedge-compression fractures at T12 often compromise the vertebral body and can encroach on the spinal canal, making the lateral profile of the body, pedicles, and posterior elements a practical teaching view for assessing stability. The mixed morphology also explains why instrumentation planning and screw trajectory differ from T11 above and L1 below in posterior fixation constructs.
