Presented in lateral profile, the left ankle and hindfoot contour is centered around the distal fibula forming the lateral malleolus, which sits posterior and slightly inferior to the distal tibia and overhangs the talar trochlea at the tibiotalar joint. Inferior to the talus, the calcaneus defines the heel prominence, with the subtalar joint line implied between them. Along the posterolateral ankle, the course of the peroneus longus and brevis tendons would track behind the fibula, while the Achilles tendon inserts posteriorly on the calcaneal tuberosity. A blue identification band encircles the ankle as a surface landmark. Lateral ankle anatomy matters because most acute ankle sprains load the anterior talofibular ligament first, then the calcaneofibular ligament, producing predictable swelling and tenderness just anterior and inferior to the lateral malleolus. That relationship is easy to teach from a true side view, since you can align surface contour with the talar dome and the fibular tip and explain why inversion injury can also irritate the peroneal tendons or destabilize the distal tibiofibular syndesmosis.
