Description
Centered on the trunk, the anterolateral abdominal wall is highlighted as a broad field extending from the costal margin superiorly to the inguinal region and pubic symphysis inferiorly, with lateral boundaries approaching the anterior axillary lines. Superficial fascia (Camper and Scarpa layers) overlies the muscular wall formed by external oblique, internal oblique, and transversus abdominis, which transition medially into aponeuroses that contribute to the rectus sheath around rectus abdominis and meet at the linea alba. Deep to the muscular and aponeurotic layers sit the transversalis fascia, variable extraperitoneal fat, and the parietal peritoneum as the internal lining. Key surface-oriented landmarks include the linea semilunaris laterally and the inguinal ligament running from anterior superior iliac spine to pubic tubercle. Appreciating these layer relationships matters when planning access to the peritoneal cavity and when interpreting abdominal wall pain or bulges. Hernias track along predictable weak zones: indirect and direct inguinal hernias near the superficial and deep inguinal rings, femoral hernias inferior to the inguinal ligament, and Spigelian hernias along the semilunar line at the lateral border of rectus abdominis. A clean depiction of the transversus plane is also clinically relevant for ultrasound-guided TAP blocks, where local anesthetic is deposited between internal oblique and transversus abdominis to cover T7 to L1 anterior rami. It is a layer problem.
