Description

Centered in the female lesser pelvis, the urinary bladder (vesica urinaria) sits immediately posterior to the pubic symphysis and anterior to the uterus and upper vagina, its dome rising superiorly toward the peritoneal reflection. Paired ureters descend along the pelvic sidewalls and course anteromedially to enter the posterolateral bladder wall at the ureterovesical junctions, establishing the superior corners of the trigone. Lateral to the bladder, the iliopsoas and adjacent pelvic wall musculature frame the visceral compartment, while the sigmoid colon and other bowel loops occupy a more superior and posterior position. Vessels and nerves track along predictable corridors. Clinical teaching often hinges on these relationships. Ureteral entry points into the bladder matter during hysterectomy and other pelvic operations because the ureter passes close to the uterine artery (the classic water under the bridge relationship) before tunneling into the bladder, a common site of iatrogenic injury. The anterior bladder wall and its retropubic (prevesical) space also define the plane exploited in midurethral sling placement and other anti-incontinence procedures, and the trigone provides a clear anatomic explanation for vesicoureteral reflux when the intramural ureteral segment is short or incompetent.
