Description
Beneath the superficial fascia of the male anterior thoracic wall, the external intercostal muscles occupy the intercostal spaces between adjacent ribs, with fibers running inferoanteriorly from the rib above to the rib below. Their digitations are arranged segmentally from the parasternal region laterally toward the midaxillary line, where the muscular portion transitions toward a thin external intercostal membrane as it approaches the sternum. Surrounding shoulder and arm musculature is kept subordinate in tone, but the deltoid and proximal biceps brachii provide lateral and superior landmarks, and the external oblique appears inferiorly as it interdigitates with the lower ribs. Fiber direction matters here. External intercostals form part of the “bucket-handle” mechanism of inspiration, elevating the ribs and increasing transverse thoracic diameter when the intercostal neurovascular bundle and thoracic cage are intact. The same intercostal spaces serve as corridors for procedures, so being clear about where muscle gives way to membrane helps when teaching chest tube placement along the midaxillary line, and when discussing why intercostal nerve blocks target the neurovascular plane inferior to each rib. Anatomy faculty can pair this plate with respiratory mechanics lectures to contrast external versus internal intercostal fiber orientation, and surgical educators can use it to introduce the layered chest wall encountered in thoracostomy, thoracentesis, and intercostal catheter insertion.
