Description

Centered on the proximal anterior thigh, the femoral triangle is outlined inferior to the inguinal ligament and lateral to the pubic tubercle, with its lateral border formed by the medial edge of sartorius and its medial border by adductor longus. Deep to the fascia lata, the floor is defined by iliopsoas laterally and pectineus medially, creating the familiar wedge-shaped interval between the abdomen and upper limb of the lower extremity. Within this space, the femoral nerve lies most lateral, the femoral artery and vein sit medial to it in the femoral sheath, and the femoral canal occupies the most medial compartment, adjacent to the lacunar ligament. Surface anatomy matters here. Clinically, this is the corridor for common femoral arterial access, where puncture ideally targets the artery over the femoral head and above its bifurcation, reducing the risk of pseudoaneurysm or retroperitoneal hemorrhage from a high stick. The relationship of the femoral vein medial to the artery explains inadvertent venous cannulation during catheter placement and guides ultrasound probe orientation when teaching landmark-based versus image-guided technique. The medial femoral canal is also the pathway for femoral hernias, which often present below and lateral to the pubic tubercle and may incarcerate because of the rigid boundaries formed by the inguinal and lacunar ligaments. Use this illustration in gross anatomy and regional anatomy courses to teach femoral triangle boundaries and the NAVL arrangement (nerve, artery, vein, lymphatics), and in procedural training materials for femoral line placement, arterial blood sampling, and femoral nerve block planning.
