Centered in the thorax, the heart sits in the middle mediastinum between the right and left lungs, with the right atrium and right ventricle positioned anterior to the left-sided chambers and the myocardium forming the thick muscular walls. Superior to the cardiac base, the trachea descends in the midline and bifurcates at the carina into the right and left main bronchi, which course inferolaterally into the hila and branch into lobar and segmental bronchial trees toward the peripheral alveolar regions. Inferiorly, the domes of the diaphragm separate the lungs and heart from upper abdominal viscera, with the stomach and proximal intestines indicated in situ beneath the left hemidiaphragm. Clinically, the tracheal bifurcation and carina are the landmark for bronchoscopy and endotracheal tube placement, and their relationship to the main bronchi explains why aspirated material more often tracks into the wider, more vertical right main bronchus.
