Description
Centered on the upper lateral abdomen, the right and left hypochondriac regions sit inferior to the costal margins and lateral to the epigastrium, framing the upper quadrants beneath the domes of the diaphragm. Superficially, the external oblique muscles are highlighted along the flanks, their fibers running inferomedially from the lower ribs toward the linea alba and iliac crest. Deep to this abdominal wall layer, the right hypochondrium overlies the right lobe of the liver and gallbladder region, while the left hypochondrium corresponds to the spleen, gastric fundus, and the splenic flexure of the colon. Clinically, this regional anatomy anchors bedside localization of pain and tenderness, separating right upper quadrant hepatobiliary patterns from left-sided splenic and gastric presentations. Cholecystitis classically produces right hypochondriac pain and guarding under the costal margin (Murphy sign), whereas blunt trauma can enlarge or lacerate the spleen in the left hypochondrium with possible Kehr sign referral to the left shoulder via diaphragmatic irritation. Surface anatomy matters. The relation of the external oblique and costal margin also guides safe incision planning and trocar placement to avoid the subcostal neurovascular bundles and to respect the upper abdominal musculature during laparoscopic access.
